Individual Therapy (Initial Intake) - $250/hr

Neuropsychological Testing -  $250/hr

Psychological Evaluations - $250/hr

Fitness for Duty Testing - $250/hr

 

Medical/Legal Evaluation - $300/hr

Full payment or co-payment is due at the time of service. Clients are responsible for all fees not covered by insurance.

Good Faith Estimate:

If clients are uninsured or plan to self-pay (without submitting a claim to health insurance), they have a right to a Good Faith Estimate.​​ The Good Faith Estimate provision of the No Surprises Act grants clients the right to predictable healthcare charges.

 

We tailor neuropsychological testing and psychological evaluations to each client and can provide an estimated range for charges once we complete intake.​​

Insurance:

Dr. Josh Clark is an in-network provider with most Pacific Source plans and will collect your co-payment at the time of service.

For all other insurance carriers, we will collect full payment at the time of service. If your health insurance offers out-of-network benefits, we are happy to provide you a detailed receipt with all the information you need to submit claims for reimbursement. Please contact your insurance company directly to confirm the details of your coverage.

 

Cancellation Fees:

We have a 2 business day cancellation policy.

 

Clients canceling with less than 2 business days' notice will automatically be charged a nonrefundable $100 fee for therapy appointments or a $500 nonrefundable fee for testing and evaluation appointments.
